Department of Physical Therapy

Admission Requirements

  1. Candidate must hold a current physical therapy license within a jurisdiction in the United States (copy of current license must be included in the application packet).
  2. Official transcript verifying graduation in physical therapy. (Original transcript verifying completion from the institution must be mailed to the Division. See address below.)
  3. Application to the University of Utah Graduate School
    Can apply on-line at http://www.sa.utah.edu/admiss/index.htm
  4. Application to the Division of Physical Therapy. A completed application to the Division of Physical Therapy includes the following:
    • the Division of Physical Therapy post-professional DPT application form
    • Physical Therapy application fee of $125
    • a current professional resume
    • a copy of current physical therapy license
    • official transcript verifying completion of physical therapy degree/graduation (may be sent under separate cover by the institution housing the PT program)
    • completion of the national Physical Therapist’s Evaluation Tool (PTET)
    • a narrative of the applicant’s practice history, description of current practice, and objectives for his/her doctoral education
