General Admission Requirements
Application to the University
Students applying with a bachelor's degree MUST apply to the University of Utah Graduate School by December 1, of the preceding year. Transfer students must apply to the University of Utah by December 1, and include a transfer summary sheet with their application to the Division of Occupational Therapy.
Application to the Division of Occupational Therapy
The application deadline for the Division of Occupational Therapy is January 15 of the year you wish to start the program. Qualified candidates will be invited for an interview consisting of group and individual interviews and a written personal statement. Interviews will happen mid to end of February. Students will know if they've been selected for admission by the end of March. Qualified students not initially selected for admission may be offered an alternate spot. Alternates will be selected to maintain the class size. The Division of Occupational Therapy does not maintain a waiting list from year to year.
A completed application includes the following:
- The Division of Occupational Therapy application with all sections completed according to directions.
- Payment of the $75 Occupational Therapy application fee.
- Two letters of recommendation using forms. One from an employer and one from an occupational therapist.
- A minimum of 2 verification forms documenting a minimum of 50 contact hours with an OT professional.
- Official transcripts from all colleges attended. These transcripts are in addition to the transcripts required for transfer or graduate applications and should include the grades for any course enrolled in–up to and including the Fall Semester of the current year. All transfer students, in addition to official transcripts from all colleges attended, should submit a University of Utah Summary of Transfer Credits.
- GRE Exam (and if required, TOEFL) Test results. The test(s) must be taken early enough so that the Division of Occupational Therapy receives the scores by the application deadline.
Incomplete or inaccurate files/forms will not be reviewed.

It is the responsibility of the student to initiate inquiries regarding receipt of all application materials.
Students may include a self-addressed-stamped postcard or envelope with his/her application materials. The Division will use this to inform students regarding the status of his/her application materials.
