Graduation Forms
Masters Degree Graduation Forms can be found at the Graduate School website. All forms should be turned in to the Division Assistant.
- Request for Supervisory Committee
- This form needs to be completed by end of fall semester 2nd year. - Application for Admission to Candidacy (Masters)
- This form needs to be completed by the end of September of the 3rd year. - Report of the Final Examination for the Master's Degree
- This form will be completed in the final academic semester (Fall, 3rd Yr) close to the Research Symposium. - Bachelors Degree Graduation Application
This form is due to the Registrar's office by November 1 for spring graduation and by February 1 for summer graduation.
