Application

Congratulations!  Everything you need to become a registered occupational therapist and spend your career helping people help themselves is right here, right now.  The Division of Occupational Therapy at the University of Utah has the faculty, curriculum, facilities and research capabilities to give you an unsurpassed professional education.

A Complete Application 

An application is considered complete when the following items are received at the Division of Occupational Therapy office:

  1. Completed application (electronic preferred)
  2. Application fee
  3. Occupational therapist reference
  4. Employer reference
  5. A minimum of 2 observation forms totaling a minimum of 50 hours (or one verification form and verification of an intro to occupational therapy course on a transcript)
  6. Copy of CPR and First Aid Certificates
  7. Official college transcripts
  8. Application to the University of Utah - transferring without a bachelors degree or as a graduate student if a bachelors degree has been or will be completed by the start of the program

Application Forms

These forms will need to be printed off and given to a therapist or employer to complete.  They will then need to be mailed in separately.
