Release of Information

This form is required for the ESS Department to release information to students over the phone or via a personal email address, other than Umail; to release academic information about a student to a third party; and for letters of recommendations to a third party from an University official (faculty member, advisor, etc). The form is required in order to observe the Family Education Rights and Privacy Act (FERPA). For more information on FERPA, visit the student handbook.

Major Declaration

In order to declare the major, students will need to complete and sign the major declaration form. This form will be given to all students at their declaring appointment and is only available on the Web site as a reference. Students must meet with an advisor to declare their major.

Academic Advising Syllabus

The academic advising syllabus was created to better help students understand their role and responsibility in the department, and to clarify the roles and responsibilities of the advisor and department. Students will need to print out a copy of the syllabus for their personal academic files. Students should reference their syllabus prior to each meeting with the ESS advisor.

Graduation Checklist

This form will need to be completed either prior or at the same time as the graduation application for undergraduate students. Students will need to fill out this form and turn in a copy to the ESS department to keep on file. It is the students responsiblity to fill out the form accurately and completely and be aware of those courses they are required to take for graduation.

Undergraduate Graduation Application

Must be completed and turned in to the graduation office two semesters before a student’s intended graduation date. All instructions are included on the form.

Petition for Consideration of Exception to Policy

This form is required for all students looking to have their transcripts altered, like removing W grades, adding a class for a past term, changing credit hours for a past term, or elective or revoking CR/NC option. Exceptions will only be made in cases involving unusual or extenuating circumstances. For more information, visit the Office of the Registrar’s Web page.

Undergraduate Leave of Absence

This form is required for any student planning on leaving the University for an Official Assignment with a nonprofit or government agency. Leaves will not be approved for medical, personal, or academic emergencies. Students who do not fill out a leave of absence form will be required to reapply to the University, and will be subject to complete any new requirements for the University, major, and/or minor.
