APPENDICES

The information in the appendices is supplemental information. You must include all pertinent information in your narrative. Please include in the appendices: publications, surveys, evaluations, transcripts from interviews, questionnaires, and any thing related to your project.

Outline your clinical protocols, procedures, policies, data collection processes, original photos (a copy must be in the narrative), and other stepwise parts of your project.
